Monitoring
Monitoring of care delivery is carried out by the following process
Daily Diary
The daily diary known as "communication sheets" is kept in the care plan and an entry made on each call by the care assistant, giving brief summary of services during the visit. These sheets are returned to the office periodically for filing in the client's file.Monthly key worker report
Badminton Care has a policy to have a named keyworker for each service user and private client that uses Badminton Care on a permanent basis.Policy Statement
Badminton Care will maintain awareness and ensure care assistants understand their role as a key worker. Ensuring a close knowledge of the needs and interests of the Service users and Private clients, ensuring the co-ordination and delivery of high standards of care and communication.Service Users/client should know what is expected of the keyworker.
Keyworker is expected to co-ordinate information and pass back to the office information regarding Health and Safety matters, monitoring care plans, and ensuring the best possible delivery of care.
A monthly report is submitted by the keyworker Care assistant for each Service user or Private client to their Care Manager at Badminton Care. Where necessary an interim report is submitted.
The Manager will action any requirements or shortfalls where appropriate. The keyworker monthly report includes and confirms:
- Paperwork is being followed
- The service user has full details of Badminton Care how to contact us
- Care plan is fully up to date
- Date of last care plan assessment
- Has the Client Service user received all services laid down in the care Plan
- Communication Sheets
- Risk assessment sheets
- Complaints Form
- Keyworker card
- Contacts
Survey
Quarterly survey will be sent to service users on receipt trends and analysis of questions agreed with Social Services Department (SSD) will be analysed 25% per qtr will be targeted.Annual Review visit
Service users will receive an annual visit from a senior member of the care staff to check that risk assessments and care delivery is being delivered to the standards expected of Badminton Care. The opportunity will be taken to ensure the service user is happy with the care delivery from Badminton Care.Courtesy Visits
Are made to high dependency Service Users on commencement of a care package by a senior member of the team. This is to ensure the service will run to the level of expectation of the Service User. Further visits may be arranged between annual reviews if there are any concerns over care delivery e.g. feedback through the keyworker or keyworker reports.Insurance
